Terms and Conditions for Therapy Services

A Guide for Clients and Practitioners

Introduction

These Terms and Conditions (“Agreement”) govern the provision of therapy services (“Services”) by [StrongHeart Warrior Project] (“the Practice,” “we,” “us,” or “our”) to clients (“you” or “your”). By engaging in our therapy services, you indicate your acceptance of these terms and conditions. Services may be delivered in person, online, or by telephone depending on client preference, therapist availability, and relevant laws or regulations.

1.1 Limitations of Service

Our practice does not provide emergency services. In the event of a mental health crisis or emergency, please contact emergency services or proceed to your nearest emergency room.

1.2 Suitability

Therapy services may not be suitable for everyone. We reserve the right to assess the suitability of the therapeutic relationship and may refer you to alternative services if deemed necessary.

2. Confidentiality

Your privacy and confidentiality are of utmost importance to us. All information shared during therapy sessions is confidential and will not be disclosed to third parties without your written consent, except in circumstances outlined below.

2.1 Exceptions to Confidentiality

Confidentiality may be breached only under the following circumstances, as required by law:

  • If there is a risk of harm to yourself or others
  • If there is suspected or disclosed abuse or neglect of a child, elder, or vulnerable adult
  • When required by a valid court order or subpoena

Your therapist will discuss any necessary disclosures with you where possible.

2.2 Record Keeping

We maintain secure records of your sessions in compliance with applicable data protection laws. You have the right to request access to your records, subject to legal and ethical considerations.

3. Appointments and Cancellations
3.1 Scheduling

Appointments are scheduled by mutual agreement. Please notify us of any schedule changes as early as possible.

3.2 Cancellation Policy

We require [24/48] hours’ notice for cancellations or rescheduling. Cancellations made with less notice may incur a cancellation fee, except in cases of emergency or illness.

3.3 Missed Appointments

Failure to attend a scheduled appointment without notice may result in a charge for the full session fee.

5. Therapist-Client Relationship

5.2 Dual Relationships

Therapists will not engage in dual relationships (business, social, or otherwise) with clients to avoid conflicts of interest.

5.3 Communication

Communication outside of scheduled sessions should be limited to administrative matters (e.g., scheduling, billing). Therapeutic content should be reserved for sessions.

6. Termination of Services
6.1 Client-Initiated Termination

You may end therapy at any time. It is recommended to discuss termination with your therapist to support a positive transition.

6.2 Therapist-Initiated Termination

Your therapist may terminate services if:

  • You violate the terms of this agreement
  • Your needs are outside the therapist’s scope of competence
  • Therapy is no longer effective or appropriate

In such cases, referrals may be provided where possible.

6.3 Transfer of Care

Upon termination, you may request that your records be forwarded to another healthcare provider, subject to your written consent.
